Consent forms
Collect digital consent before appointments — health history, photo release, signature, and custom questions.
Consent forms let you collect health disclosures, photo release agreements, custom waivers, and a digital signature from clients before or at their appointment. Available on all plans.
Setting up your consent form: 1. Go to Dashboard → Consent Forms → Settings tab 2. Toggle Enable Consent Form on 3. Write your Acknowledgment text — a short statement clients confirm before signing (e.g., "I confirm the information above is accurate and I consent to the services requested.") 4. Add custom questions using the builder: - Click Add Question - Choose type: Text (free-form answer) or Yes / No (checkbox) - Mark as Required if the client must answer before submitting - Drag to reorder, or click the trash icon to remove 5. Click Save
Choosing which services require consent: Enabling consent turns it on for your business — but you decide which services actually require it. Go to Dashboard → Services, edit a service, and use the "Require consent form" toggle. This lets a shop require a signed waiver for, say, tattoos while skipping it for a quick consultation. New services require consent by default once your business template is enabled; turn it off for the ones that don't need it. (This is separate from intake forms — consent is a signed legal waiver; intake is just questions. You can use either, both, or neither per service.)
What clients fill in: - Date of birth - Allergies - Medical conditions - Photo consent (opt-in checkbox) - Age confirmation (18+ or supervised minor) - Your custom questions - Typed or drawn digital signature on a canvas pad
When the form is shown: - Booking flow: When a client books a service that requires consent, they're prompted to fill in the consent form during booking - Walk-in Kiosk: After a walk-in is added for a service that requires consent (Business or Enterprise kiosk), the form appears inline on the tablet for the client to complete in person
If your business template is disabled, or the booked service doesn't require consent, the step is skipped automatically. Clients are never blocked from completing a booking just because a consent form is skipped.
Viewing submitted forms: 1. Go to Dashboard → Consent Forms → Submissions tab 2. You'll see a list of every submission with: - Client name and email - Date signed - Link to view the full form - Download PDF button
Each PDF is professionally formatted with all answers, the acknowledgment statement, and the client's signature.
Enterprise consent hub: If your business is on the Enterprise plan, your org dashboard has a central Consent Forms hub (Dashboard → Organization → Consent Forms) that shows signed forms across your entire team roster in one place.
Pro tip: Turn on consent forms before your first high-risk service (color, chemical treatments, tattooing, injections, waxing). Having a signed record on file protects you if a client later claims they weren't informed of risks.
